Trellis' recently released Client Pledge promises to "Act on your feedback and let you know what we did". 

We stive to deliver quality services that meet your needs.  In our efforts to keep making improvements that align with our Values and to "live into" our Pledge, Trellis invites our clients and their family members to complete a survey. 

Surveys are available at each Trellis location and in the community with our Outreach Workers. You can also choose to submit a Survey by printing off a hard copy pdf, filling it in and returning by mail to the attention of:
