Family Support, Advocacy and Education Services

Trellis offers family members support, advocacy and education services in a variety of individual and group sessions. In this program, we support you not your family member. As a caregiver/family member, you will become our client. You must follow our Intake Procedure before being able to access our program. Once the Intake process is completed you will meet with our Family Support and Education Worker to develop a plan that addresses your unique requirements.

Who Qualifies?

  • Caregivers and family members living in Wellington and Dufferin
  • As a caregiver/family member, your loved one who is ill must be 18 years of age or older
  • Your loved one who is ill is not suffering from early Dementia or Alzheimer's disease
  • Your loved one does not necessarily have to fall under the moderate to severe mental health category
  • Your loved one does not have to be accessing Trellis' services

How to Access Help?

All caregivers/family members that wish to receive services through the family program must complete the lntake Process.  Call Trellis' Intake Services at 519-821-3582.
