  • your family member shows signs of behavioural changes associated with dementia.

Referrals are usually made through a family physician or other physician and occassionally by a community service provider.  Since the service is provided in partnership with your family physician, it is important that she/he be involved in the referral from the start.  For more information, or if you do not have a family physician and wish to contact our service, please call            519-821-2060 / 1-800-265-7723.
