Trellis can help

The Seniors' Mental Health Service is a unique and innovative program designed to provide specialized service to seniors aged 60 and over (or younger if there is an age-related component) living with mental illness or dementia in Wellington and Dufferin.  Primarily an outreach service, visits are made where the person resides (home, retirement home, or long term care home). 

The team consists of mental health workers, nurses, social workers, and community psychiatrists.  We work closely with your family physician and other professionals to partner in your care. 

Trellis Services for Seniors Can

  • provide assessment, consultation and treatment from a team of professionals and community partners, including geriatric specialists;
  • provide resources to support your goals and enhance your wellness;
  • provide education and health teaching to you and your family/caregivers;
  • provide follow-up visits to monitor the effectiveness of recommendations, including medications and supportive strategies;
  • assist you to meet your goals.
